Come and be part of  the Complex Clinical Care Network team (CCCN) as a Māori Health Clinical Assessor, we are a team of multi skilled health professionals who invests in a proactive system of care to wrap services around people identified with complex needs, to ensure they stay healthy and well in their own communities for as long as possible.

The position will be based in Greymouth but you will be expected to work collaboratively from a kaupapa Māori perspective with Poutini Waiora, West Coast Māori Health team and other relevant agents to proactively target the most complex Māori clients across the West Coast.

As a central player in the team you’ll find out what is needed and see that your Māori clients receive appropriate and timely support by applying kaupapa assessment tool in conjunction with the interRAI assessment tool.

CCCN is community focused and is a leader in a client centred restorative model for clients with complex needs and is a point of referral to support delivery of integrated services.

You will be an experienced registered Occupational Therapist, Physiotherapist, Social Work or Registered Nurse and bring enthusiasm and a desire to work collaboratively to respond to the broad ranging needs of the Māori population on the West Coast. Excellent communication, computer and organisational skills are a prerequisite of this varied role.

A current full New Zealand driver’s licence is essential.
